How To Make An Appointment

To schedule an appointment with a Physician, call the following: (860) 963-2133 or contact us through your patient portal. Important:

  • If your insurance company requires a referral, please obtain it from your pcp prior to your appointment.
  • If your physician asks you to return for a follow-up appointment, please schedule this before you leave the office or call the office if you are being discharged from the hospital.
  • A minimum notice of 48 hours for appointment cancellation is appreciated.
Our offices are open Monday through Thursday from 9:00am to 5:00pm, Friday from 9:00am to 4:00pm.
  • Films, reports, test results and previous records which pertain to this appointment
  • Insurance card(s)
  • Co-payment required by your insurance company
  • List of current medications and dosages
  • Comfortable clothing to access area of examination
  • In addition: If Workers’ Compensation, then please also bring:
    • Copy of first report of injury
    • Claim #, Insurance Company and Adjustors, name, address and phone number. For more information you may go to: State of CT Workers’ Comp

Prescription Refill Requests FAQ

Managing your prescription needs is an important part of your care.

 

If you need a prescription renewal sooner, please call the office during normal business hours:

  Available Monday – Friday,  9am- 3pm

 

Please be sure to let the clinical assistant know what pharmacy you use so that your medication will be ready as soon as possible.

 

Please try renew your prescriptions at the time of your office visit.

 

Please make every effort not to call after hours or at the end of the day for a prescription renewal.

 

It is our duty to make every effort that you are given the prescription you need and can only do so when we have your clinical information to review. It is always best that the provider who is caring for you be involved in ordering or renewing any prescriptions.

Yes. Requests for refills require planning on your part. 

 

Please avoid waiting until you are out of your medicine before calling us.

 

We appreciate three (3) business days to process medication refill requests. Refill requests take 48 hours to process.

Your 24/7 Patient Portal is an easy way to contact your provider, to request a New Medication! We will review after-hour requests on the next business day (M-F) Requests for new medications, or one different from what you are now taking, may take up to 48 hours to process.

  • If you believe you are having an allergic reaction to a medication, please call our telephone nurse immediately at (860) 889-7345, or go to the Emergency Room.
  • Prescriptions will not be refilled at night, on weekends, or on holidays.
  • If you are taking more than the prescribed dose, the prescription will not be refilled.
  • If you are taking pain medication or an anti-inflammatory prescribed at another office, let us know.
  • Lost or stolen prescriptions and/or medications will not be replaced.
  • Medications will not be provided to patients who have not been seen by the Physician in the past 6 months.

Please Note: As part of our transition into Orthopedic Partners, there have been some changes to the insurance networks in which we are able to participate. 

We will make all efforts to contact you prior to your appointment if we are aware that you have an insurance plan that we are no longer in network with. Please call our office prior to your next appointment if you have any questions about whether we are in or out of network with your specific insurance plan.

 

Payments

Paying for care today is a complex and sometimes overwhelming process. To facilitate this process it is important that the patient, spouse, parent or other responsible party be prepared by making available the information necessary for our office to file a claim on your behalf.

 

HEALTH INSURANCE

You must present your insurance cards and any referral forms required by your insurance company at the time of your appointment. If your insurance company has certain requirements that you do not follow, they will make you responsible for the bill. Please ask the staff at the front desk to update your insurance information whenever you are in the office.

If we do not participate with your insurance, we can submit a claim on your behalf – but remember that the insurance company may not pay the claim and you will be responsible for the bill.

 

WORKERS COMPENSATION

If you are injured at work or through an employment activity, you MUST provide the required information from your employer prior to your appointment so that we can validate the payment source.

  • Please provide the following:
  • Insurance Carrier
    Name
    Claim address
  • Case Manager/Adjuster
    Name
    Phone
  • Claim #
  • Date of Injury

Failure to do so may result in your employer or Workers Compensation Carrier denying your claim. 

 

MEDICARE

As a participating provider with Medicare we are pleased to submit your Medicare claim and any secondary insurance claim forms for you.

 

SELF PAY

We require that all services be paid at the time they are rendered. We will consider payment schedules but in the event the patient fails to honor the payment schedule the fee for services provided will revert back to the full charge allowed by law.

 

CO-PAYS & DEDUCTIBLES

Medicare and all other insurance carriers require that we collect the co-payments and deductibles as outlined in your insurance policy. It is our policy to collect these charges at the time of service.
